On House or Senate hearings, they usually say "Do you swear or affirm...", so I'm sure courts do the same.

The bible is not mandatory. One may affirm without it.

Hummm do they even use a bible in court at all anymore???
The one and only time I had to testify on a jury case all they did was asked me to raise my right hand to promise that I would tell the truth and the whole truth ect.. on questions that I would be asked... |

Swearing on a bible is long gone in most courts. I've been in courts many times (no, only very rarely as a defendant) and I have NEVER seen anyone bring out a Bible to swear on.
