tip for a server that does a good job?

depends on the total...im a very good tipper...like last weekend we went to eat and the bill was 18 dollars...i gave her 6 dollars as a tip

20-30 percent.
Once I didn't leave any tip. We weren't waited on for over 45 minutes. The food was cold. Oh and there was only a table of 4 in the same section. |

I waited tables for years I start at 20% & go up or down depending on service. I usually sit a long time & talk, if I am there a long time I add more because the server could have flipped that table & gotten another tip. UNLESS they try to rush me out, then they get less.
I am a good waitress & usually make 25-30 % tips. |

Oh my pet peeve in restaurants... when you pay with a large bill & the server asks if you want change...

depends how many people are with me, its normally just 2. so at best, Ill leave 6 dollars. average is 5.50 and then below depending.
I dont do percents %%  its not a hard job, they dont need more then 6 dollars to carry food and refill drinks. 
my pet peeve. salad, bread, and soup should come 10 minutes before the main dish. If they cant do that right, and they bring them out together with my main dish  , I might not even leave a tip.
